Document templates and official form guides
Download editable NOTG templates, find current government forms, and read practical guidance before you sign. Each resource explains its common purpose, what to gather, and what to confirm with the organization receiving your document.
Start with the receiving organization
A template is a starting point, not a guarantee of acceptance. Before you invest time in drafting, ask the receiving government office, school, bank, court, consulate, employer, or other organization whether it requires a particular form, exact wording, supporting evidence, original ink signatures, witnesses, notarization, commissioning, translation, authentication, or apostille.
Our house templates are provided in editable Word format with a PDF reference copy. Where an Ontario or Canadian government body publishes a prescribed form, we link to the official source instead of altering or rebranding it. That helps you reach the current version and preserves the form exactly as issued.
Smart Template Search
Describe the document or situation using a few ordinary words—for example, “my child is travelling with an aunt” or “two spellings of my name.” This private, on-page catalogue search only filters the reviewed resources below. It does not send your words to an AI service, draft legal language, recommend a legal strategy, or provide legal advice. Do not enter passport numbers, SINs, health information, financial details, or other sensitive personal information.

Important use notes
These templates and guides provide general information only and are not legal advice. Requirements vary by recipient, jurisdiction and individual circumstances. Confirm the required form and wording with the receiving organization before use. Do not sign an affidavit or statutory declaration until you are before an authorized commissioner or notary. Downloading or using these materials does not create a lawyer-client or other professional-client relationship, and acceptance is not guaranteed.
For wills, powers of attorney, separation agreements, court strategy, immigration advice, or any document that can materially affect legal rights, obtain advice from an appropriately licensed professional. A notary or commissioner can administer an oath or affirmation and witness eligible signatures, but does not automatically advise on the legal effect of your document.
